Aged Care Case Manager / NDIS Support Coordinator

  • Fixed Term (6 months) Part Time and Fixed Term Full Time 
  • 64 hours per fortnight and 80 hours per fortnight (including monthly ADO)

About the role

Community Interlink provide access to support services for frail older people, people with disabilities and their carers. The funding used in the program will help to provide in home supports for consumers to maintain safety, choice, dignity and independence in their home for as long as possible.

This position forms part of the Case Management/Planning team responsible for providing Planning and Case Management services to our aged care package recipients and NDIS consumers. Case Management/Planning is predominantly a technical role confined to supporting clients to utilise their funds within the guidelines effectively, this includes: Care Planning, Assessment, Coordination of supports, Goal setting, monitoring effectiveness, and formal reviews.

To be successful at this role you will need to be able to think outside the box and have an understanding of local services available.  You will have an understanding of the social model of health and support people to have access to their local community.

You will have the opportunity to work collaboratively with other health professionals to maintain and build upon the capabilities of your clientele.

You will work with people to identify and meet their goals in this consumer directed care model. 

We are looking for

  • A Tertiary qualification in an appropriate Health and/or Human Services field, or extensive, relevant experience.
  • Sound experience of the knowledge and skills required in working with frail older people, people with disabilities and their carers.
  • Well-developed interpersonal and communication skills, including the ability to initiate and maintain contacts with a broad range of service providers and community agencies.
  • Knowledge of the Case Management approach to service delivery in a community setting.

As per the NDIS Quality and Safeguards Commission prospective employees must be checked against the National Disability Insurance Scheme Worker Screening Database.  You will be requested to complete and submit the National Disability Insurance Scheme Worker Screening Check via Service Victoria. Your Check will be assessed to determine your eligibility to work within a NDIS role.  If you do not receive clearance from the NDIS Worker Screening Unit or you do not agree to participate in the Scheme you cannot be engaged by a NDIS provider. Appointments to this role are also subject to satisfactory Victorian ‘Employee’ Working with Children’s Checks. Completion of a Commonwealth of Australia Statutory Declaration is required prior to commencement.

  • Essential : Knowledge of the Case Management approach to service delivery, in a rural community setting.
  • Essential : The ability to exercise independent judgement and innovative problem solving skills in day-to-day case management.
  • Essential : Sound case management skills, with a focus on capacity building, planning and financial monitoring skills.
  • Essential : Satisfactory completion of Victorian 'Employee' Working with Children Check and completion of a Commonwealth of Australia Statutory Declaration will be required from prospective candidates prior to commencement of employment. Do you agree to completing these requirements?
